Previewing a template

The Preview button opens the published form exactly as respondents see it. You can click through it, but please do not submit sample answers.

Copying a template to your Drive

Use this template opens Google's "Make a copy" screen. Sign in to your Google account, confirm, and the form appears in your Drive with every question, section and theme setting. The guide How to copy a Google Form template covers every step, including linking the copy to a response spreadsheet.

The privacy block

Every form ends with a privacy section. The text in it is a placeholder: replace it with your own notice before you share the form. Keep the required "I have read the privacy notice" checkbox and leave the communications checkbox optional and separate.
